As of March 1, 2011, the following changes have been implemented in order to remain in compliance with the U.S. Federal Passport Laws.
- A non-applying parent or guardian consenting to the issuance of a passport for a minor child is required to complete Form DS-3053. The non-applying parent or guardian must also provide a photocopy of the front and back side of his or her ID with Form DS-3053. THE STATEMENT OF CONSENT IS NOT COMPLETE WITHOUT THIS PHOTOCOPY.
- Only ‘1’ passport photo is required with Form DS-11 and Form DS-82. The color photo, measuring 2” x 2”, must be taken within the last 6 months, (reflect current appearance), with a plain white or off-white background.
