Requirements for a New Passport

For detailed information on the requirements for a new passport please visit the U.S Department of State website.  As a reference, below is some general information related to applying for a new passport.

 You must apply in person and complete form DS-11 if any of the following is true:

  • You have never had a U.S. passport;
  • You are under age 16;
  • Your most recent passport was issued more than 15 years ago; or
  • Your most recent passport was lost or stolen

When applying for a passport, please provide the following:

Proof of citizenship

  • Original or certified copy of your birth certificate (must include parent’s full names)
  • Certificate of naturalization (if applicable)
  • You must also provide a black and white photocopy of your proof of citizenship. We can provide this service for you at a charge of $1.00 per page.

Proof of identity (one of the following):

  • Valid driver’s license 
  • State identification card
  • You must also provide a photocopy of the front and back of your proof of identity. We can provide this service for you at a charge of $1.00 per page.

Temporary IDs are not accepted as proof of identity.

Passport photo

  • Recent color photo (background must be completely white)
  • Front view – eyeglasses must be removed
  • 2 inches X 2 inches
  • Photo can be obtained at the Clerk’s office for $12.00

ALL APPLICANTS MUST PAY THE CLERK OF COURT A PROCESSING FEE OF $35.00 (CASH OR CREDIT/DEBIT ONLY)

3.5% PROCESSING FEE APPLIED TO DEBIT/CREDIT CARD PAYMENTS.

Effective November 1st, 2016, customers applying for or renewing their passport must remove eyeglasses for their passport photo.

Estimated Time to Receive Passport

1 – 2 day delivery service is available for the Department of State to return your passport book once issued for an additional $19.53 payable to the Department of State.

Passport Card

You may apply for a passport card, which may only be used for land and sea travel between the United States and Canada, Mexico, Bermuda and the Caribbean.

Passport Reapplication for Name Change

If your name recently changed and your passport was issued less than 1 year ago, use form DS-5504.

If your name recently changed and your passport was issued more than 1 year ago, use form DS-82.

Passport Renewal

The Clerk’s Office does not need to process passport renewals.

Passport renewals MUST be done by mail if you meet all of the following criteria: 

  • Your passport is submitted with your application
  • Your passport is undamaged (other than normal “wear and tear”)
  • Your passport was issued in the last 15 years
  • You were at least 16 years of age when it was issued
  • Your passport was issued in your current name (or you can document your name change with an original or certified copy of your marriage certificate, divorce decree, or court order)

If you qualify, complete form DS-82 and return to the address listed on the form.

If any of the above statements do not apply to you, you must apply in person using the DS-11 form.

Passport Applications for Minors

Passport applications for minors (under age 16) use form DS-11 and provide the following:

  • Proof of citizenship in the form of a birth certificate
  • Driver’s license, if applicable
  • Most recent passport, if applicable
  • Parent(s)/Guardian(s) must submit evidence of their relationship to the minor applicant
  • The minor must appear in person

Both parents must appear together to sign the application form. If only one parent is appearing, the absent parent must submit a notarized Statement of Consent (form DS-3053) and a photocopy of the front and back of his/her driver’s license. The consent form cannot be more than three months old.

When applying for a passport for a newborn, the parents must provide proof that they have applied for a Social Security Number for the child in question.

How long does it take to get a passport?

I need to get my passport as quickly as possible, is special expedited processing available?

Yes. You must pay the U.S. Department of State an additional $79.53 surcharge. Please visit www.travel.state.gov for current expedited processing times.

What if I need a passport in less than 14 days?

You must make an appointment with the Miami Passport Agency by calling (877) 487-2778. You must provide an itinerary or proof of tickets or a letter from an employer on company letterhead, and you must travel to the Miami Passport Agency.

I had a passport before, but it has expired. Can I simply renew it?

Yes, If it was issued within the past 15 years, and if you were 16 years or older when the passport was issued. You must also be able to submit the expired passport. Form DS-82 is available viawww.travel.state.gov for renewals. When using this form, YOU MUST APPLY BY MAIL.

Can I get passport forms online?

Yes, you can obtain forms by visiting www.travel.state.gov.

What countries require a Visa?

You may visit www.travel.state.gov.

Can I get a U.S. Passport if I am not a U.S. Citizen?

No. You must contact the nearest consulate for your country or contact Immigration & Naturalization Services.

My passport has been lost, stolen, or mutilated. What must I do?

You must complete a new application, with new pictures, birth certificate and complete
an ”Affidavit of lost or stolen” passport. The forms are available in the Clerk’s office or viawww.travel.state.gov.

I am a convicted felon. Can I get a passport?

The Federal Passport Agency will determine if you are eligible. Contact the passport agency at 877-487-2778.

If I owe money for Child Support, am I eligible to get a passport?

Public Law 104-193 stipulates that a person being in arrears on child support by an amount exceeding $2,500 is ineligible; however, you should contact the passport agency at 877-487-2778.

What are the requirements for a passport photograph?

You must provide one (1) 2 x 2 passport photo. It must be a color photo, must be identical with a full frontal view with a white or off-white background. It must show your current appearance (no uniforms, hats or head gear). Photo must have been taken within the past six (6) months.

Passport photos of customers wearing eye glasses will not be accepted, except under special circumstances.  Please visit www.travel.state.gov for details.

Can I sign my application in advance?

No. An oath must be administered before signatures are taken.

Because of identity theft, I don’t want to list my Social Security number, do I have to list it?

Yes. Social Security numbers are mandatory.

If I don’t have a certified copy of my birth certificate, how can I get one?

You must contact the Office of Vital Statistics where you were born or visit www.vitalchek.com to order your birth certificate on-line. 

Florida residents can receive birth certificates through the Florida Department of Health or Tax Collector’s Office

Once issued, how long will my passport be good for?

An Adult passport Book/Card is valid for ten (10) years.
A child’s passport Book/Card is valid for five (5) years.

Lost Passport Due To Hurricane Ian

According to travel.state.gov ,  The passport application fee waiver for victims of Florida Hurricane Ian is valid until September 28th, 2025. The file search fee waiver is valid until March 28th, 2024. You can only qualify for a fee waiver at this time if you lost your passport during Hurricane Ian. If you’ve never had a U.S. passport, or you lost your passport as a result of a disaster other than one listed on travel.state.gov, or you lost an expired passport, then you do not qualify for a fee waiver for a replacement passport.
